We collect, store, and use PHI to provide HBOT and wellness services, schedule appointments, and communicate with you regarding your care and billing.
We ensure that PHI is protected through administrative, technical, and physical safeguards—comparable to HIPAA requirements.
Treatment:
Share necessary information with our staff or contracted clinicians to deliver HBOT and related services.
Coordinate with other healthcare providers at your request.
Payment:
Provide PHI to billing and insurance entities (e.g., http://Authorize.net) to process payment or insurance claims.
Contact you regarding outstanding balances and payment options.
Healthcare Operations:
Perform quality improvement, case management, training, and audit functions.
Manage and improve our services.
Legal & Regulatory:
Disclose PHI when required by law (e.g., subpoena, court order, public health authority).
Report abuse, neglect, or domestic violence as required by applicable law.
Business Associates:
Provide PHI to third-party service providers (e.g., Google Forms, Calendly, http://Authorize.net) under Business Associate Agreements ensuring PHI confidentiality.
Right to Inspect & Copy:You may request access to your PHI in electronic or paper form. We may impose a reasonable fee for copying.
Right to Amend:If you believe PHI is incorrect or incomplete, you may request an amendment. We will review and respond within 60 days.
Right to an Accounting of Disclosures:You can request a list of disclosures of your PHI made for purposes other than treatment, payment, or healthcare operations.
Right to Restrict Uses & Disclosures:You may request restrictions on how your PHI is used or disclosed. We are not required to agree, but if we do, we will comply.
Right to Confidential Communications:You may request that we communicate PHI by alternative means or locations (e.g., only by email).
Right to Receive a Paper Copy of This Notice:You can request a paper copy even if you have agreed to receive it electronically.
